California law changed on January 1, 2024, related to the mandated reporting of suspected elder or dependent adult abuse. These changes impact the reporting of suspected abuse taking place in long-term care facilities.

Supervisors do not need additional permission to use these forms in the context of clinical supervision. Each form may be digitally shared in its entirety so long as it is unaltered and not part of a commercial product. Forms should be carefully reviewed in the context of all legal, ethical, and administrative policies and requirements for supervision prior to use, to ensure it is appropriate to the specific context.
Evaluation of Appropriateness for Video Supervision (California)
This form is based on the requirements established in Assembly Bill 1758 (2022) for video-based supervision of California prelicensed MFTs, PCCs, and CSWs. The form is required in all work settings and must be completed within 60 days of the onset of supervision. Version last updated August 2022.
